Electronic health records (EHRs) are a reality of clinical practice today and have posed new challenges to communicating safe and effective care. When an adverse event occurs, the documentation or lack thereof will be placed under a microscope by the plaintiff’s attorney. How would your documentation fare under such scrutiny?
